The Brantford Brant Primary Care Council


The Brantford Brant Ontario Health Team will not succeed without the support of the local primary care community. Primary care providers in Brantford Brant are forming the Brantford Brant Primary Care Council, which will provide a cohesive voice of primary care and lead change in our community.

Why are we organizing?l

          • The health system is transforming with the creation of ONTARIO HEALTH TEAMS and there is an opportunity to strengthen primary care.
          • Primary care is essential to high performing healthcare systems.
          • Together our voice is stronger than as individuals, for improving things within an OHT context and for any things outside of it.
          • We are often weighed down by challenging responsibilities, while lacking a network of supports to help us.
          • We hear what our patients need and want it to be easier for them to access care.
          • We want to promote digital solutions.
